Shelton Care Management- Geriatric Care Management

For years both of our parents suffered from serious illnesses. Mom had Alzheimer’s and dementia and dad had serious lung and heart issues. Mom was out of control and a danger to herself and others. Every 6 months dad seemed to be in the hospital. They did not have a qualified primary care physician that recognized or addressed their special needs.



Then we found Patty. She immediately bonded with the family and orchestrated a plan for how best to care for our parents (which included finding a physician and attorney who understood geriatric special needs) and how to survive the intense stress. Mom has since passed away, but under Patty’s care and management, dad, now 82, has not been in the hospital for almost 2 years. With Patty’s supervision, he is learning to manage his health and is leading a productive life.



I don’t know what we would have done without her. (We don’t live in the same town as our parents.) We are very grateful for her compassionate, professional services and through the years, her friendship.
