Primary Care
Methods: Subjects were referred from primary care clinics and failed conservative treatment. Subjects received a diagnostic ultrasound examination, then ultrasound-guided injection of 25% dextrose with lidocaine into the area of tendinopathy until they were satisfied with treatment. The primary outcome measure was a three-part visual analogue scale (VAS; baseline and mean of 45 weeks after start of treatment) for pain at rest, activities of daily living (ADL) and during sport. Secondary outcomes included segmental ultrasound examinations assessing tendon hypoechogenicity (area and severity score), neovascularity (severity score) and the presence or absence of intratendinous tearing and calcification, irregularities of cortical bone and thickness.
Family Practice

Emergency Care
In 2022 I collaborated with several colleagues in the publication of a case report of a Marine veteran and former firefighter who had failed multiple medication trials and trials of customary therapy approaches. Clinically important improvements rapidly occurred in the PTSD severity scale (PTSD checklist), quality of life, and an anxiety and depression scale, and improvements were stable through 22 weeks. Anxiety benefit was observed within 20 minutes of injection. This treatment method, which uses a small needle injection just under a superficial muscle of the neck on each side, is well tolerated, and has potential as a supplementary treatment for standard of care treatments, and potentially for emergency care. Unlike stellate ganglion block, there is no anesthetic needed (only dextrose) and it can be done on both sides. Empirical results suggest this pattern and degree of improvement is typical. A randomized controlled clinical trial is to begin in late 2023.

Neurology
All these studies were performed at the Orthopedic Research Laboratory at Mayo Clinic, in conjunction with the department of neurology. The goal was to make an animal model of carpal tunnel syndrome by thickening the carpal tunnel ligament (transverse carpal ligament), flatting the median nerve to create carpal tunnel. The author’s hypothesis that non-inflammatory progressive transverse carpal ligament (or equivalent in animal) proliferative thickening (fibrosis) will lead to eventual median neuropathy, is supported by these studies. It is important to observe that there were no inflammatory cells and dextrose concentration was only 10%, confirming that this growth was not a result of inflammation but rather was a result of a proliferative effect of dextrose without inflammation.

This was a single injection study of participants with cubital tunnel syndrome (compression of the "funny bone nerve, also known as the ulnar nerves" as it travels across the elbow. Ultrasound guidance was used to hydrodissect about the ulnar nerve with either D5W or a steroid (triamcinolone acetonide), a total of 30 mg, mixed with normal saline. Pain improvements were similar and substantial in both groups. The ulnar nerve size (swelling) decreased more in the dextrose group. Here is that abstract.
MRI
•Candidates: 6 months of pain, failure of 3 months of conservative treatment, with positive MRI findings and grade II to IV OA. These participants qualifed as having chronic pain issues, and had received a resasonable trial of conservative treatment.

Orthopedics
6. Research on dextrose prolotherapy is the least affected by drug company or device company bias because there are no profit motives to publish positive results. Research is typically done by physicians sacrificing their time and personal finances. Despite that challenge, high quality trials have been published in many areas and are in process in others (see research tab). Despite modest sample sizes, numerous rigorous randomized controlled trials have reported a clinically meaningful effect of dextrose prolotherapy in 11 areas (knee osteoarthritis, temporomandibular dysfunction, rotator cuff tendinopathy, lateral epicondylosis, wrist pain, finger/thumb osteoarthritis, sacroiliac pain, hip osteoarthritis due to hip dysplasia, Osgood-Schlatter disease, Achilles tendinosis, and plantar fasciosis.) An example of the importance of treating pain sources both inside and outside the joint is seen in patients that still have pain after a joint replacement because they still have pain sources outside the joint itself. For this reason, whenever we talk about treating joint such as a knee or a shoulder for example, we’re talking about how to inject the joint itself and also the key ligaments and tendons around the joint. More recently we are also treating some of the nerves around the joint. By doing so we address every source of pain for a more complete pain relief.

Design: This was a randomized study with supine position injection of up to 8 ml of 13.5% dextrose into iliopsoas and adductor tendon insertions, and 8 ml intraarticular, (except in Crowe III and IV), and lateral decubitus injection of the gluteus medius, minimus and piriformis insertions every 21 days until pain reduction by 75% or a maximum of 6 sessions versus 12 weeks of supervised progressive resistive exercises according to American College of Sports Medicine guidelines followed by home exercise plan.

Of the Chinese University of Hong Kong and colleagues reviewed ten randomized controlled trials of dextrose prolotherapy for TMJ dysfunction. Dextrose prolotherapy was significantly superior to placebo injections in reducing TMJ pain at 12 weeks, and up to 1 year in studies with longer follow-up.
